In his hands Khonsu usually carried a crook and flail as well as either djed or was sceptre. Sometimes he was also falcon-headed god, similar to Re, Horus or Montu. Khonsu was often portrayed a either a mummified human, similar to Osiris or Ptah, whose head was topped with a full lunar disk on his head, not unlike the more famous solar disk. He notably became associated with healing and childbirth, and after supposedly healing king Ptolemy IV Philopator (244-204 BC) from an illness, he became seen as a protector of the pharaoh who could drive away evil spirits. Later on though, likely after becoming the son of Amun and the god of the Moon, he became a more revered deity.

In the oldest myths, notably the "Cannibal Hymn" of the Pyramid Texts, Khonsu was often portrayed as a bloodthirsty deity, who catches lesser gods and hands them over to a bloodthirsty deceased king who absorbs them. Sometimes Khonsu was also considered the son of Sobek, the crocodile-god of the Nile, and Hathor, goddess of fertility, while others connect him as potential son of Osiris. Most commonly Khonsu is seen as the son of Amun, a god of creation, and Mut, an important mother goddess, and the three were often worshipped as the powerful triad of Thebes, similar how Osiris, Isis and Horus were also worshipped as a triad.
